FireMed Program:

Open enrollment is from October 1 – December 31

All FireMed members as of August 2019 were mailed a letter. To view a copy of this letter, please click HERE.

After payment and application during the enrollment process have been made, a receipt will be emailed to you. Please keep this receipt for your records as proof of enrollment. Membership cards are no longer being issued. Membership cards are available for purchase of $25 at Canby Fire.

We encourage anyone interested in this annual subscription to visit the patient portal for more information. Brochures, applications, and consent forms are all available through the website and can also be accessed above.
